CLIENT CONSENT, LIABILITY WAIVER & TERMS OF SERVICE

By booking and receiving services from Nettle & Neem Herbal Head Spa LLC ("Nettle & Neem"), I acknowledge and agree to the following terms and conditions:

1. ACKNOWLEDGMENT OF SERVICES

I understand that Nettle & Neem provides non-medical scalp and head spa services, including but not limited to scalp cleansing, scalp massage, herbal and botanical rinses, hair treatments, aromatherapy, relaxation techniques, and related wellness services.

I understand that these services are intended for relaxation, scalp wellness, and cosmetic purposes only and are not intended to diagnose, treat, cure, or prevent any medical condition.

2. HEALTH DISCLOSURE

I certify that I have disclosed any known medical conditions, allergies, sensitivities, injuries, scalp disorders, skin conditions, recent surgeries, pregnancy, or other health concerns that may affect my ability to safely receive services. If you are pregnant or breastfeeding, please let us know before your service. We will customize your treatment using pregnancy-appropriate herbs and will avoid certain botanicals and modalities as an added precaution.

Our services require guests to lay down flat for approximately 30, 60, or 90 minutes, depending on the service selected. If your healthcare provider has advised you to avoid lying on your back or remaining in a reclined position for extended periods, please follow their recommendations when selecting a service. You may wish to choose a shorter treatment or postpone your visit until it is appropriate for you.

If you have any questions about whether a treatment is suitable during your pregnancy, we encourage you to consult your healthcare provider before booking.

I agree to inform Nettle & Neem of any changes in my health condition prior to receiving services.

Nettle & Neem reserves the right to refuse, modify, or discontinue services if a condition is identified that may make treatment unsafe.

3. HERBAL & PRODUCT ALLERGY DISCLAIMER

Nettle & Neem utilizes natural herbs, flowers, teas, botanical extracts, essential oils, and professional hair care products during services.

While every effort is made to provide safe and appropriate treatments, allergic reactions and sensitivities may occur. I understand that natural ingredients may still cause irritation, allergic reactions, redness, itching, discomfort, or other adverse effects.

I accept full responsibility for informing Nettle & Neem of any known allergies, sensitivities, or ingredient restrictions before treatment begins.

4. ASSUMPTION OF RISK

I voluntarily choose to participate in services provided by Nettle & Neem and understand that participation involves certain inherent risks, including but not limited to:

• Allergic or sensitivity reactions to products, herbs, flowers, teas, essential oils, or other ingredients used during treatment
• Temporary redness, irritation, itching, or discomfort
• Hair texture, color, or scalp changes
• Minor physical discomfort associated with massage techniques or treatment procedures
• Slips, trips, falls, or other accidents occurring on the premises

I knowingly and voluntarily assume all risks associated with receiving services.

5. RELEASE OF LIABILITY

To the fullest extent permitted by Oregon law, I release, waive, and hold harmless Nettle & Neem Herbal Head Spa LLC, its owner, employees, contractors, agents, successors, and representatives from any claims, demands, damages, injuries, losses, liabilities, costs, or expenses arising out of or related to my participation in services, except where such liability cannot be waived by law.

6. RESULTS DISCLAIMER

I understand that individual results vary and that Nettle & Neem makes no guarantees regarding scalp health improvement, hair growth, hair retention, reduction of hair loss, improvement of scalp conditions, or any specific cosmetic outcome.

7. PERSONAL PROPERTY

I understand that Nettle & Neem is not responsible for the loss, theft, or damage of personal belongings brought onto the premises.

8. EMERGENCY MEDICAL CARE

In the event of a medical emergency, I authorize Nettle & Neem to contact emergency medical services on my behalf.

I understand that I am solely responsible for any costs associated with medical treatment, emergency services, or transportation.

9. MINORS

Clients under the age of 18 must have a parent or legal guardian review and sign this agreement prior to receiving services.

The parent or legal guardian acknowledges and accepts all terms of this agreement on behalf of the minor.

10. AGREEMENT

I certify that I have carefully read and fully understand this Client Consent, Liability Waiver, and Terms of Service. I acknowledge that I am voluntarily agreeing to these terms and understand that this agreement is binding upon me, my heirs, personal representatives, executors, administrators, and assigns.
